Chapter 3: Administer enteral nutrition,
📚 6 Topics
⚑ What you will be able to do

By the end of this chapter, you will be able to:

  • Safely put on the correct personal protective equipment (PPE) by following proper work procedures.
  • Identify the most suitable enteral nutrition route for a client based on their specific condition.
  • Accurately prepare enteral nutrition feeds tailored to the client’s needs.
  • Administer enteral nutrition feeds safely and correctly according to the clinical nutritional manual.
  • Monitor a client’s tolerance to enteral nutrition feeds using the Kenya National Clinical Nutrition and Dietetics Reference Manual.
  • Document the administration of enteral feeding accurately in line with clinical nutrition and dietetics guidelines.

Mastering these skills will help you provide safe, effective nutrition support to clients, making a real difference in their health and recovery.

Enteral nutrition is a critical intervention for patients who cannot meet their nutritional needs through oral intake but have a functioning gastrointestinal tract. Administering enteral nutrition safely and effectively requires understanding the appropriate protective measures, selecting the correct feeding routes, and recognizing clinical indications. Nutrition and dietetics professionals in Kenya, especially in hospitals and rehabilitation centers, play a pivotal role in managing enteral feeding to prevent complications and ensure optimal patient outcomes.

3.1 PPEs

Proper use of Personal Protective Equipment (PPE) is essential for dietetic professionals involved in administering enteral nutrition to minimize infection risks and ensure patient safety. In Kenyan healthcare settings such as county hospitals and private clinics, adherence to PPE protocols protects both patients and healthcare workers during feeding tube placement, feeding preparation, and administration.

3.1.1 Types of PPEs

Understanding the various types of PPEs used in enteral nutrition administration allows dietitians to implement appropriate infection control measures. The choice of PPE depends on the procedure stage and the risk of exposure to bodily fluids or contaminants.

Gloves

Gloves provide a barrier against pathogens during handling of feeding tubes, preparation of enteral formulas, and feeding administration. In facilities like Nairobi Women’s Hospital, using sterile gloves during tube insertion prevents cross-contamination, while clean gloves suffice for routine feeding administration.

Masks

Surgical masks protect healthcare workers from respiratory droplets and reduce the risk of contaminating sterile equipment. For example, during insertion of nasogastric tubes at a county referral hospital, masks help prevent infection transmission between staff and patients.

Aprons and Gowns

Fluid-resistant aprons or gowns shield clothing and skin from spills or splashes of enteral feeding solutions or gastric contents. In wards with high patient turnover, such as pediatric units, disposable gowns ensure hygiene and reduce cross-infection.

Eye Protection

Goggles or face shields protect the eyes from accidental splashes of feeding solutions or gastric contents, especially during tube placement or aspiration procedures. This protection is vital in surgical wards where enteral feeding is initiated postoperatively.

Hair Covers and Shoe Covers

Hair covers prevent hair contamination of feeding equipment, while shoe covers maintain cleanliness in sterile feeding preparation areas. Though less frequently used, these are important in specialized feeding preparation rooms in major hospitals like Kenyatta National Hospital.

Practice Questions

  1. Explain the role of gloves and masks in preventing infection during enteral nutrition administration. (6 marks)
  2. Identify four types of PPE used in enteral feeding and describe their specific protective functions. (8 marks)

Chapter Summary

This chapter explored the essential personal protective equipment required for safe enteral nutrition administration, detailing types such as masks, gowns, gloves, and appropriate footwear. It then examined various routes of enteral nutrition, including mechanical options like nasogastric and nasojejunal tubes, as well as surgical routes such as gastrostomy and jejunostomy tubes. The discussion continued with the different enteral nutrition feeds available, highlighting standard formulas tailored for specific conditions like renal, pulmonary, and diabetes. Various methods of administering enteral feeds were described, including continuous drip, intermittent, and bolus feeding techniques. Emphasis was placed on the importance of monitoring and evaluating a patient’s tolerance to enteral nutrition to ensure effectiveness and safety. Finally, the chapter addressed the critical role of accurate documentation and proper dissemination of information related to enteral feed administration within healthcare settings.
